When visitors leave comments on the site we collect the data shown in the comments form, and also the visitor’s IP address and browser user agent string to help spam detection.
Media
If you upload images to the website, you should avoid uploading images with embedded location data (EXIF GPS) included. We currently do not have an upload function, however we reserve the right to add this function to any of our forms at any time. Downloading images from our site. Our patient images in the Gallery Section of our site and on specific pages are property of Dr. Pamela Kulback and K2 Restorative Medicine. Any usage or re-publishing of those images is not allowed without permission. Content created on this website may be subject to copyright of K2 Restorative Medicine and their brand partners as well as DGM Global Marketing Solutions. Copying certain content and re-publishing of unique content is not allowed.

How long we retain your data and contact info
If you leave a comment, the comment and its metadata are retained indefinitely. This is so we can recognize and approve any follow-up comments automatically instead of holding them in a moderation queue.
For users that register on our website through any form or sign up tool (if any), we also store the personal information they provide indefinitely. Visitor comments or forms may be checked through an automated spam detection service and also be exported to Mailchimp and Mail Munch and be used by our company exclusively for advertising such as specials, events, and news about new procedures and services. You can always opt out by unsubscribing from receiving those emails.

Refund and Exchange Policy
Gift Cards – Gift Cards are not redeemable for cash and have no cash value – No refund or transfer to another person other than the person named on the Gift Card is allowed. Gift Cards must contain an order number if purchased through an online store.In the case of a physical Gift Card the Gift Card (or receipt for Gift Card with amount) must be signed by designated authorized personnel employed exclusively by K2 Restorative Medicine.
Pre-Paid packages – All prepaid packages for procedures and services at K2 Restorative Medicine are not eligible for a refund. Packages cannot be transferred or traded for another service after purchase is complete. Exception to the rule is proof of unexpected Military Deployment out of state and/or country lasting 12-months or longer in which case a partial refund will be provided for any unused portion of the package. Individuals facing deportation or incarceration do not qualify for a refund. Prepaid packages are valid 12-months from the date of purchase. We reserve the right to change our policy as it relates to pre-paid packages at any time. Check back to view our policy frequently.
Products – Products cannot be refunded once they leave our premises. Please check expiration dates prior to purchase and prior to leaving our premises. Opened used products cannot be exchanged for another product unless the third party vendor of the product has an open exchange or return policy. Exceptions can only be made by Dr. Pamela Kulback not an employee at K2 Restorative Medicine & Med-Spa. If the product was purchased as a Gift to someone else and has not been opened by that person, he/or she may only exchange the product for another product. No refund is allowed.
Specials and Offers
Specials cannot be retroactively applied to previously performed procedures and are subject to change at any time. Specials are not honored on previously quoted procedures. Specials are subject to change without notice due to limited supply and limited availability of our providers. Specials must contain a valid expiration date, current coupon code, or proof of validity of the offer via screenshot with the publishing date of the offer. Graphic images with expired offers found on search engines such as Google, Bing, or Duck Duck Go via the image search tool will not be honored. Social Media offers must contain expiration dates or posting dates in order to be honored.
Patient Treatments and Services
All procedures at K2 Restorative Medicine are nonsurgical in nature. *Results for procedures and services vary from patient to patient and are not guaranteed.
